Service
Users referred to Hetton Home Care Services by the City of
Sunderland Social Services Department will be visited by one
of our senior staff prior to the commencement of the service.
At this visit a risk assessment will be completed. Your service
user plan will be agreed with you and a copy left for your
information. You will be given a pack containing contact numbers
for Hetton Home Care Services, office and out of hours. Services
Users Information File, our Complaints Procedure, and your
Service User Plan.
You
will be given the name of the Home Support Worker/s who will
be providing your care and times of their visits will be agreed.
We will explain to you what happens when your normal home
support worker is not available.
Your
social worker/care manager will review your package of care
on behalf of Social Services. A senior member of staff from
Hetton Home Care Services will also visit you at least once
a year to review your care package. If you feel your circumstances
have changed at any time and you would like to discuss these
changes you should contact your social worker/care manager.
If this is a problem please speak to your home support worker
who will help you to make contact.
